Systems of Care Wraparound Supports
Vision
Our vision is for children, youth, and families on the Yakama Nation Reservation and surrounding areas to be able to easily access a collaborative network of systems and supports that will meet their needs. We aim to accomplish this by implementing a Systems of Care approach that will meet the needs in each of the communities we serve. We work towards creating system level changes to improve health outcomes for our children, youth, and families.
What is Wraparound?
Wraparound is a way to help families by making plans that focus on their strengths and needs. Each family creates personalized plans that cover everything they need help with and what they’re good at. They then choose a team of people to help them make the plan work; these people can include family members, friends, teachers, or others they trust. They can also get support from the community by using available resources. This teamwork and community help are vital for their plans to succeed and for improving life for the family.
What to Expect
During the Team Process, individuals and families can expect the following:
- Discovery of strengths and needs
- Identification of supports
- Individualized plan for the youth and family, prioritized by the youth and family
- Supported transition from formal wraparound process to reliance on natural supports
Services
The Systems of Care Program offers the following services:
- Youth and family supports
- Care coordination
- Skill building
- Family and youth peer support
- Parenting programs
- Cultural supports
Eligibility Requirements
Youth and families seeking to use System of Care services must be:
- American Indian or Alaska Native
- 21 years old or younger
- Experiencing serious emotional or behavioral challenges
- In need of or receiving services from more than one system, such as child welfare, education, justice services, mental health, health, recreational, etc.
- In out-of-home (OOH) placement, returning from OOH placement, or at risk of OOH placement
- In need of youth and family skills training and therapeutic interventions
- Aware of and agree to receive Wraparound supports or referrals
- Living on or near the Yakama Nation Reservation
